JOB SUMMARY
Answer Health incorporates ambulatory pharmacists as part of their quality team to achieve improved patient experience of care, health of populations, reduced costs, and increased provider satisfaction in the care delivered to patients. The pharmacists provide comprehensive medication management for a wide variety of patients with chronic health conditions. They work closely with Answer Health member practices and may collaborate with various members of a patient’s care team including physicians, care managers, behavioral health providers, or others in a primary care setting.

ESSENTIAL FUNCTIONS
The roles and responsibilities of this position support the culture, mission, vision, values and key strategies of our organization. To perform this job successfully, an individual must be able to perform each essential function satisfactorily.

Duties and responsibilities
1. Work within Answer Health primary care practices to provide Medication Therapy Management services to patients.
2. Make appropriate pharmacotherapy recommendations and adjustments to providers based on current guidelines, laboratory values, vitals, co-morbid conditions and other patient-specific factors.
3. Provide patient follow up on medication adjustments and new medications started. Monitor for anticipated effects, drug side effects, adverse reactions, drug-interactions, and toxicities.
4. Use motivational interviewing to identify patient-specific barriers to medication treatment.
5. Expand patient access to medications and remove cost-barriers via Patient Assistance Programs.
6. Lower healthcare costs through identification of duplicate therapies, alternative therapies, and expected pharmacologic response.
7. Increase medication compliance and decrease patient pill burden as clinically appropriate.
8. Reduce use of high-risk medications through provider education and patient counseling.

Skills and abilities:
1. In-depth knowledge of current national guidelines for pharmacologic management of chronic conditions: hypertension, hyperlipidemia, diabetes, asthma, congestive heart failure, and COPD.
2. Up-to-date on evidence-based pharmacotherapy and guidelines.
3. Identify and interpret relevant laboratory values.
4. Ability to use and document in multiple EHRs.
5. Knowledge of community pharmacy processes (Part D billing, prior authorization process, generic vs. brand coverage/availability, and brand name co-pay cards).
6. Excellent written/verbal communications and interpersonal skills.
7. Ability to work autonomously, with strong time management, priority setting, and organizational skills.
8. Ability to develop positive, longitudinal relationships with practice managers, providers, patients, and work colleagues.

Physical Effort: The job includes limited physical demands, typical of the job that include regular walking, standing, stooping, bending, sitting, typing, use of phone, and some lifting of light objects.

Mental & Visual Fatigue: Work may produce mental/visual fatigue. Detailed, interactive work requiring alertness and concentration for sustained periods of time; operation and full attention to a computer.

EDUCATION, LICENSURE, AND CERTIFICATION

Required Qualifications:
1. Doctor of Pharmacy (PharmD) graduate degree from an accredited pharmacy college recognized by the American Council of Pharmaceutical Education (ACPE).
2. Valid State of Michigan pharmacist license.

Preferred Qualifications:
1. Previous experience with Medication Therapy Management (MTM).
2. PGY-1 +/- PGY-2 residency with ambulatory care focus or work experience in ambulatory care/primary care practice.
3. Experience working with/in multiple electronic medical records (including but not limited to, EPIC, eClinical Works, Athena, Greenway, Allscripts) or ability to learn quickly.
4. Previous work experience in community pharmacy.
